MRCP PACES Station 5: Mastering Rheumatology Cases

Ask recent PACES candidates what appeared in Station 5 and, alongside the thyroid nodules and diabetic feet, one theme comes up again and again: joints. Rheumatology is Station 5 gold. The signs are visible at the bedside, the histories are rich, the prescribing is genuinely dangerous if done carelessly, and the counselling separates a safe future consultant from a candidate who has merely memorised lists. This guide walks through the format, the frameworks, the classic cases and the safety points that convert a borderline performance into a clear pass.

Why Examiners Love Rheumatology in Station 5

Rheumatology uniquely tests everything PACES wants to sample in a short consultation:

  • Pattern recognition — the distribution of joint involvement often hands you the diagnosis.

  • Functional assessment — asking what the patient can no longer do is medicine at its most human.

  • Therapeutics with teeth — methotrexate, allopurinol and steroids are among the most examined drugs in the entire exam.

  • Chronic disease communication — explaining a lifelong diagnosis and building a shared plan.

  • Systemic screening — the hands are often the window to the kidneys (SLE), the eyes (scleritis), the lungs (RA-related fibrosis) and the heart.

Station 5 Format: What You Must Deliver in 10 Minutes

Since the PACES23 revision, Stations 2 and 5 each contain two 10-minute brief clinical consultations. Each encounter is a compressed version of a real clinic appointment: gather a focused history, examine as appropriate, synthesise, investigate, and communicate a safe plan.

A minute-by-minute blueprint that reliably works:

One Framework for Every Joint

Whatever the joint, the sequence is the same: Look, Feel, Move, Function, then connect the dots.

  1. Look — swelling, deformity, muscle wasting, erythema, scars, rashes.

  2. Feel — temperature, tenderness (joint line versus entheses), effusion, crepitus, nodules, tophi.

  3. Move — active before passive; note any restriction pattern.

  4. Function — grip, writing, doing up buttons, walking. Function scores marks in every domain.

  5. Connect the dots — before concluding, deliberately expose and inspect: nails (pitting, onycholysis), extensor surfaces, scalp and natal cleft (psoriasis), elbows (rheumatoid nodules, gouty tophi), face (malar rash, telangiectasia), and hands again for sclerodactyly.

A brief GALS-style screen is acceptable, but in Station 5 depth beats breadth: a crisp, well-narrated examination of the hands plus one targeted region impresses far more than a rushed whole-body sweep.

The Seven Scenarios You Must Own

1. Newly Diagnosed Rheumatoid Arthritis — Methotrexate Counselling

The setup: a 45-year-old woman with three months of symmetrical hand pain and stiffness; examination shows active synovitis.

Key delivery points:

  • Early aggressive treatment prevents deformity — say this out loud; it is the central biological message of the modern RA era.

  • DMARDs are started early, with methotrexate first-line at a weekly dose (commonly starting 7.5–15 mg then titrated), plus folic acid at least 5 mg weekly to reduce toxicity.

  • Counselling checklist: weekly dosing (emphasise the day of the week), alcohol moderation, blood monitoring with FBC, LFTs and U&E, report fever, infection or mouth ulcers promptly, effective contraception during and for a period after treatment, avoid live vaccines, offer annual influenza and pneumococcal vaccination, and avoid trimethoprim or co-trimoxazole (dual antifolate toxicity).

  • Escalation: combination DMARDs or biologic and JAK inhibitors after inadequate response — biologic screening includes latent tuberculosis and hepatitis B and C.

  • Surround the drug with the team: physiotherapy, occupational therapy, podiatry, treat-to-target review with a composite disease activity score, and smoking cessation (smoking worsens disease and blunts treatment response).

Examiner favourite: what is the single most important safety point when prescribing methotrexate? Answer: weekly administration — daily dosing is a classic fatal prescribing error. Say it unprompted.

2. Gout — Managing the Flare and Starting Urate-Lowering Therapy

  • Acute flare options: NSAID (with gastroprotection), low-dose colchicine (500 micrograms once or twice daily — far better tolerated than older high-dose regimens), or a short oral corticosteroid course; choose by comorbidity.

  • Diagnose clinically: serum urate can be normal during a flare, and a normal level never excludes gout. Joint aspiration showing negatively birefringent monosodium urate crystals is definitive when uncertain.

  • Urate-lowering therapy (allopurinol first-line): start low (100 mg daily; 50 mg in significant CKD), titrate every few weeks to a target serum urate below 360 micromol/L, or below 300 if tophi are present, and continue long term. Modern NICE guidance (NG219) supports starting during an acute flare with ongoing anti-inflammatory cover — and at the very least, never stop urate-lowering therapy during a flare.

  • Give flare prophylaxis (colchicine or NSAID) for up to six months while urate-lowering therapy is initiated and titrated.

  • Address drivers: alcohol (especially beer and spirits), sugar-sweetened drinks, obesity, diuretics (review with the prescribing specialty), CKD and cardiovascular risk.

  • Interaction trap: allopurinol with azathioprine risks profound myelosuppression; caution also with warfarin.

3. Polymyalgia Rheumatica — and the GCA Question You Cannot Miss

  • Presentation: over 50, bilateral shoulder and pelvic girdle pain and stiffness, worse in the morning, difficulty rising from a chair or lifting arms; ESR and CRP typically raised.

  • Treatment: prednisolone around 15 mg daily with a dramatic, rapid response that is itself near-diagnostic; taper over one to two years; add bone and gastrointestinal protection for prolonged steroids.

  • The station-winning skill: actively screen for giant cell arteritis — new headache, scalp tenderness, jaw claudication, visual symptoms, limb girdle symptoms with claudication. Any visual symptom is an emergency: start high-dose corticosteroid immediately without waiting for temporal artery imaging or biopsy, and arrange same-day specialist review. Delay costs sight.

  • Mention glucocorticoid-induced osteoporosis prevention: calcium and vitamin D, bone density assessment, bisphosphonate where risk is high.

4. Inflammatory Back Pain — Axial Spondyloarthritis

  • The pattern: onset under 45, back pain over three months, morning stiffness beyond 30–60 minutes, improves with activity rather than rest, night pain, and a good NSAID response.

  • Linked features that earn marks: enthesitis (Achilles and plantar heel pain), dactylitis, previous or current uveitis (ask about red painful eyes), psoriasis, inflammatory bowel disease symptoms, and family history.

  • Investigations: inflammatory markers, HLA-B27, and MRI of the sacroiliac joints — plain films may be normal early in the disease course.

  • Management: NSAIDs plus structured physiotherapy and exercise as the foundation, early rheumatology referral, biologic and JAK therapy for persistent disease, smoking cessation, and screening for uveitis and cardiovascular risk.

5. Osteoarthritis — Management That Follows Guidelines

  • Core treatment for all: education, exercise (quadriceps strengthening for knees — as effective as many drugs), and weight loss where relevant.

  • Topical NSAIDs are first-line pharmacotherapy for hand and knee OA; oral NSAIDs at the lowest effective dose with gastroprotection and caution in older adults (renal, cardiac and GI risk).

  • Paracetamol alone is no longer routinely recommended — a guideline-savvy line that signals current reading.

  • Intra-articular corticosteroid injection for flares, aids and supports as needed, and referral for joint replacement when persistent symptoms despite maximal therapy impair function.

6. Osteoporosis — The Fragility Fracture Consultation

  • A fragility fracture (from standing height or less) in anyone over 50 should trigger assessment: DEXA, fracture risk tools, and a search for secondary causes — thyroid disease, glucocorticoids, myeloma in the older patient, vitamin D deficiency, hypogonadism, alcohol.

  • A prior hip or vertebral fracture usually warrants treatment without waiting for DEXA.

  • First-line: alendronate 70 mg weekly with calcium and vitamin D if intake is inadequate.

  • Counselling that earns marks: take on an empty stomach with a full glass of water and stay upright for at least 30 minutes, report oesophageal symptoms, mention rare osteonecrosis of the jaw alongside routine dental care, plan review at around five years for possible drug holiday, and treat falls risk in parallel — vision, home hazards, medication review, vitamin D.

7. Connective Tissue Disease — The Young Woman with Joint Pain and Fatigue

  • Screening questions: photosensitive rash, oral ulcers, hair loss, Raynaud phenomenon, dry eyes and mouth, pleuritic chest pain, foamy urine, miscarriage or thrombosis history (antiphospholipid syndrome).

  • Investigations: ANA as a sensitive but non-specific screen; anti-dsDNA and low complement supporting active SLE; urinalysis with quantified protein at every lupus review to catch nephritis early; CK for myositis; Scl-70 and anticentromere antibodies with nailfold capillaroscopy in suspected scleroderma.

  • Management: hydroxychloroquine for nearly all SLE patients with baseline and periodic retinal screening, sun protection, vaccination and pregnancy planning (hydroxychloroquine is usually continued in pregnancy to reduce flares), and urgent referral for renal involvement.

  • Scleroderma extras: Raynaud management, close blood pressure monitoring (renal crisis presents with accelerated hypertension), lung function surveillance, and reflux treatment.

Prescribing Safety: The Marks That Decide Borderline Candidates

  • Methotrexate: weekly, with folic acid. State the day of the week out loud. This is the most classic safety point in PACES.

  • Allopurinol: start low, titrate to target, continue through flares, avoid with azathioprine, caution with warfarin.

  • Corticosteroids: prescribe gastroprotection and bone protection for prolonged courses, check glucose, warn about infection risk and never stop long-term steroids abruptly.

  • Biologics and JAK inhibitors: screen latent tuberculosis and viral hepatitis first; no live vaccines.

  • NSAIDs: cover renal, cardiovascular and gastrointestinal risk in one sentence before prescribing in any older adult.

  • Colchicine: low dose now preferred; toxicity escalates with renal impairment and interacting drugs.

Common Pitfalls Seen From the Examiner Side of the Table

  • Examining joints meticulously but never once asking about function, work or independence.

  • Missing psoriasis because you did not look at the scalp, natal cleft or umbilicus.

  • Missing tophi behind the elbow or on the ear.

  • Screening nobody for GCA in a polymyalgia case.

  • Prescribing methotrexate without stating weekly dosing or offering folic acid.

  • Starting allopurinol with no prophylaxis and no titration target.

  • Delivering a beautiful differential with no safety-netting and no follow-up plan.

  • Running out of time because the history had no structure — the minute blueprint above prevents this.

A Two-Week Rheumatology Sprint for Station 5

  • Days 1–3: Frameworks — joint examination sequence and the pattern table; practise the hands examination aloud daily.

  • Days 4–8: One case per day drawn from the seven scenarios; full timed 10-minute runs; write the single mark-winning sentence for each case.

  • Days 9–11: Prescribing drill — methotrexate, allopurinol, steroids, biologics, NSAIDs, with monitoring schedules straight from current guidelines.

  • Days 12–14: Mixed simulation with feedback — a study partner, a course circuit or AI patient practice for sheer volume; record yourself and listen for filler words, jargon and missed patient concerns.

Practise with whoever is available: colleagues challenge your clinical reasoning, while AI patient simulations excel at repetition — rehearsing the counselling phrasing that sounds unnatural the first ten times until it becomes yours.

Final Word

Rheumatology cases reward the candidate who combines a sharp eye, a structured minute-by-minute plan and genuine prescribing safety. Walk in with the pattern table in your head, the methotrexate sentence on the tip of your tongue, and a reflex for function and red flags — and Station 5 stops being an obstacle and becomes your opportunity to show the examiner exactly the registrar you already are.
